This Statement of Purpose will be reviewed annually and whenever there is a significant change to the practice, its services, registered manager, locations or CQC registration.
1. ABOUT THE CROSS KEYS PRACTICE
The Cross Keys Practice is a General Medical Services (GMS) GP practice providing comprehensive primary medical services to a registered population of approximately 15,000 patients.
The practice operates from a main site in Princes Risborough and two branch surgeries in Chinnor and Lincoln House. Together, the three sites provide accessible primary healthcare to patients living in these communities and the surrounding areas.
The practice is provided and run by The Cross Keys Practice, a partnership comprising five GP partners:

- Dr Amy George
- Dr Dushan Chaminda Hettiarachchi
- Dr Rangah Niranchanan
The GP partners are supported by five salaried GPs and a wider multidisciplinary team of clinical, administrative and support staff.
The Cross Keys Practice is committed to providing high-quality, safe, effective and compassionate care. We seek to develop long-term relationships with our patients and ensure that our services are responsive to individual needs and to the changing health requirements of our local population.
2. OUR PURPOSE
The purpose of The Cross Keys Practice is to provide safe, effective, compassionate and accessible primary healthcare to our registered population.
We aim to support patients throughout their lives by promoting health, preventing illness, diagnosing and treating disease, managing long-term conditions and coordinating care with other healthcare professionals and services.
We recognise that high-quality general practice requires strong clinical leadership, effective teamwork, good governance, continuity of care and a commitment to listening and learning.
Our practice aims to provide care that is centred around the individual patient while making effective use of available resources and embracing appropriate developments in healthcare and technology.
3. OUR AIMS AND OBJECTIVES
The Cross Keys Practice aims to:
- Provide safe, effective and evidence-based healthcare.

5. SERVICES WE PROVIDE
The Cross Keys Practice provides primary medical services to its registered population across its main site and two branch surgeries.
Services include, as appropriate to individual patient need:
- General medical consultations and assessment.
- Diagnosis and treatment of acute illness.
- Management and monitoring of long-term conditions.
- Preventative healthcare and health promotion.
- Health checks and screening.
- Childhood immunisations and vaccination programmes.
- Adult immunisations and vaccination services.
- Family planning and contraception services.
- Maternity and midwifery services.
- Women's health services.
- Sexual and reproductive healthcare.
- Assessment and treatment of disease, disorder and injury.
- Appropriate surgical procedures.
- Diagnostic and screening procedures.
- Medication reviews and medicines management.
- Referrals to secondary care and other appropriate healthcare services.
- Care planning and support for patients with complex or multiple healthcare needs.
- Support for patients with frailty and age-related healthcare needs.
- Palliative and end-of-life care where appropriate.

- Remote and digital consultations where clinically appropriate.
The availability of individual services may vary between sites according to clinical need, staffing, facilities and operational arrangements.
6. CQC REGULATED ACTIVITIES
The Cross Keys Practice is registered with the Care Quality Commission (CQC) to provide the following regulated activities:
- Maternity and midwifery services
- Family planning
- Treatment of disease, disorder or injury
- Surgical procedures
- Diagnostic and screening procedures
The practice provides services for everyone within its registered patient population, subject to clinical need, appropriate assessment and the scope of the practice's CQC registration.
The regulated activities are carried out across the practice's main site and branch surgeries, subject to the facilities, staffing and clinical arrangements available at each location.
The practice will maintain this Statement of Purpose so that it accurately reflects its current CQC registration, regulated activities and locations.
7. PEOPLE WHO USE OUR SERVICES
The Cross Keys Practice provides services to approximately 15,000 registered patients.
We provide care to patients across all age groups and seek to respond appropriately to the individual needs of our population.
Our services are intended to meet a wide range of healthcare needs, including:
- Acute and minor illness.
- Long-term and chronic conditions.
- Multiple and complex healthcare needs.
- Preventative healthcare needs.
- Screening and health promotion needs.
- Mental health needs.
- Disability and accessibility needs.

The registered manager is responsible for the management of the regulated activities provided by the practice and works with the GP partners and wider practice leadership team to maintain appropriate arrangements for clinical governance, patient safety, quality improvement, risk management and compliance with regulatory requirements.

The Cross Keys Practice is led by its five GP partners, supported by five salaried GPs and the wider practice team.
We recognise that effective leadership and governance are essential to the delivery of safe, effective and high-quality healthcare.

- The way in which regulated activities are delivered.
The practice will ensure that the Statement of Purpose remains accurate, current and consistent with its CQC registration.
